Analysis of CT Finding and Clinical Aspect of Chronic Subdural Hematoma

Abstract
The findings of computed tomography(CT) and clinical manifestations were analyzed retrospectively in twenty one cases of chronic subdural hematoma.
Also fluid samples of these chronic subdural hematoma patients were analyzed in laboratory and compared with CT density findings. Experimental models containing various concentration of hemoglobin, albumin and iron were made and their CT densities were examined.
The results were summarized as follows
1. There were 19 men and 2 women in twenty one cases of chronic subdural hematoma. The peak incidence was noted in 6th and 7the decade of age.
2. Headache was the most common symptom complained in nearly all cases (95.2%) and hemiparesis was the most common neurological sign(61.9 %) .
3. The older the age became, the more the volume of hematoma appeared to increase, but the less the extent of midline shifting.
4. CT density of all hematomas were classified into hypodense, isodense, mixed dense and hyperdense lesions, noted in 23.6%, 29.2%, 23.6% and 23.6 of cases respectively, distributing rather evenly.
5. The laboratory values of red blood cells, hematocrit, and hemoglobin in hematoma samples were very variable, however their values were correlated positively with the CT densities. No significant correlationship was found between the total values of protein or iron and the CT densities.
6. In vitro laboratory models, a linear correlation had been established between the hemoglobin concentration and the CT density, but not between the levels of albumin or iron and the CT density.
